Everything you ask for is already there: Each time a new version of an
structure-item is generated, the new version number, a time-stamp and
the username (if available) is assigned.
When you have a look at the version of a structure-item, you will see
all this information presented there. If I am not wrong you should
even see the date of the last change when editing a page.
